Hak Pesakit & Waris

  • Dilayan dengan hormat, maruah dan tanpa diskriminasi, selaras dengan undang-undang dan polisi hospital.
  • Keselamatan semasa menerima rawatan dan penjagaan di fasiliti hospital.
  • Mendapat maklumat yang jelas, tepat dan mudah difahami mengenai diagnosis, pilihan rawatan, risiko/manfaat, jangkaan hasil, serta kos berkaitan.
  • Memberi informed consent (persetujuan termaklum) atau menolak rawatan tertentu mengikut undang-undang dan polisi semasa.
  • Privasi semasa konsultasi, prosedur dan rawatan, serta kerahsiaan maklumat peribadi/rekod perubatan.
  • Melibatkan diri dalam keputusan penjagaan, termasuk hak untuk mendapatkan pendapat kedua (second opinion).
  • Pengurusan kesakitan yang sewajarnya dan rawatan paliatif mengikut keperluan klinikal.
  • Keperluan keagamaan, budaya dan nilai peribadi diambil kira selagi tidak menjejaskan keselamatan/penjagaan.
  • Akses kepada waris/penjaga/peneman mengikut garis panduan lawatan yang berkuat kuasa.
  • Mengemukakan pertanyaan, aduan atau maklum balas dan menerima maklum balas sewajarnya.
  • Disediakan maklumat berkaitan peralihan penjagaan, discaj, susulan dan kontinuiti rawatan.
  • Akses kepada maklumat bil/kewangan yang telus mengikut peraturan.

Tanggungjawab Pesakit & Waris

  • Memberi maklumat perubatan dan peribadi yang tepat, lengkap dan terkini kepada penyedia perkhidmatan kesihatan.
  • Mematuhi pelan rawatan yang dipersetujui serta nasihat klinikal; maklumkan jika enggan/menolak rawatan.
  • Mematuhi peraturan hospital, termasuk waktu lawatan, kawalan jangkitan, keselamatan dan penggunaan kemudahan.
  • Menjaga adab sopan, menghormati hak pesakit lain, pelawat dan petugas kesihatan.
  • Mengurus temujanji: hadir tepat masa atau memaklumkan pembatalan/penjadualan semula lebih awal.
  • Menyelesaikan tanggungan kewangan yang berkaitan rawatan mengikut garis panduan.
  • Melindungi barangan peribadi dan tidak membawa bahan/barangan terlarang.

Patient & Family Rights

  • To be treated with dignity and respect, without discrimination, in line with laws and hospital policy.
  • To receive care in a safe environment with appropriate safety measures in place.
  • To be given clear, accurate and understandable information about diagnosis, treatment options, risks/benefits, expected outcomes and related costs.
  • To give informed consent or to refuse certain treatments as permitted by law and current policy.
  • To privacy during consultations, procedures and care, and to confidentiality of personal/medical information.
  • To participate in care decisions, including the right to seek a second opinion.
  • To appropriate pain management and palliative care according to clinical needs.
  • To have religious, cultural and personal values reasonably accommodated where safe and feasible.
  • To have access to family/guardian/companion according to prevailing visiting guidelines.
  • To ask questions, submit complaints or feedback, and to receive a timely response.
  • To information supporting discharge, follow-up and continuity of care.
  • To transparent billing/financial information in accordance with regulations.

Patient & Family Responsibilities

  • To provide accurate, complete and up-to-date medical and personal information to healthcare providers.
  • To follow the agreed treatment plan and clinical advice; inform staff if refusing treatment.
  • To comply with hospital rules, including visiting hours, infection control, safety and use of facilities.
  • To be courteous and to respect the rights of other patients, visitors and staff.
  • To manage appointments by arriving on time or notifying cancellations/rescheduling in advance.
  • To settle financial obligations related to care as guided.
  • To safeguard personal belongings and not bring prohibited items/substances.
